If you are participating in this year's One-Act Play, please use this form to select your meal options for the following days:

• 3/3: Tuesday's Home Performance #1 Dinner at 5:30pm (Taco Cabana boxes)
• 3/9: Monday's Zone Contest Lunch at TBD (Chick-fil-A "limited menu" provided by the Booster Club at no charge)
• 3/9: Monday's Zone Contest Dinner at 7:30pm (Domino's pizza provided by the Booster Club at no charge)
• 3/24: Tuesday's Home Performance #2 Dinner at 5:30pm (Chick-Fil-A "full menu").

If you would rather bring your own food for any or all days, please select the "None - I will bring my own meal" option for the meal(s) that you intend to skip.

If we advance to district, another meal choice form will need to be filled out for lunch and dinner meals on Thursday, March 12.

ALL MEALS MUST BE PAID FOR BY 6:00PM ON SATURDAY, FEBRUARY 28. You can "Pay in Full" now by credit card/Booster Credits or "Pay Later" by placing a check or cash in an envelope labeled with your name and "OAP meal payment" in the BLACK BOOSTER BOX in the Theatre hallway by 6:00pm on Saturday. Please make checks payable to "CRHS Theatre Booster Club".

• Extensions: Please email meals@theatreattheridge.com if you would like to request a payment extension.
• Financial Assistance: If you are a student who requires financial assistance for meals, please select the applicable box on the order form.
• Nonpayment: If you order meals, but do not pay by Saturday and have not emailed for an extension or requested financial assistance, we will not be able to place your order for Tuesday's meal. The Boosters will have Sun Butter & Jelly sandwich supplies, chips, and water for anyone who does not have food or forgets to make their payment.
